- Your Name and Age
- Your Badge Number
- Name of Your Panel and Rating: E (Everyone), T (Teen), M (Mature), AO (Adult Only)
- Short Description of Your Panel (250 characters with spaces maximum) – If chosen, this will be printed verbatim
- Duration of Your Panel
- Preference of Day for Your Panel: Friday, Saturday, or Sunday
- Preference of Time for Your Panel: Morning (9am – Noon), Afternoon (Noon – 5pm), Evening (5pm – 9pm), Night (9pm – Midnight), Late Night (After Midnight)
- Additional Comments/Technical Requirements – Do you have Audio/Visual needs?
